#### Anal Cancer Rates Are on the Rise in Houston
Researchers at the University of Texas in Houston have recently published a study in the journal of National Cancer Institute showing increased incidence of anal cancer. Anal cancer is closely related to HPV, human papilloma virus, infection. The virus can be prevented by vaccination but more than 50% of Houstonians are not vaccinated.

Anal cancer and hemorrhoids (almorranas in Spanish) have many symptoms in common including fresh blood per rectum, pain around the anus, itching and feeling a lump near the anus. It is important to be evaluated by a hemorrhoid specialist to differentiate both hemorrhoid from anal cancer. A colonoscopy or anoscopy is often recommended to confirm the diagnosis.

#### Anal Fissures and Hemorrhoids: What’s in Common?
20% of Houstonians with symptomatic hemorrhoids have an associated anal fissure. The co-existence of both disorders is not surprising. Both anal fissures and hemorrhoids (Almorranas in Spanish) are caused by constipation and diet poor in fiber. Both diseases cause significant discomfort leading to decreased quality of life.

Hemorrhoids are dilated blood vessels of the lower rectum. An anal fissure is a tear of the thin moist lining of the anus. Both anal fissures and hemorrhoids are associated with increased anal sphincter muscle tone. Nitroglycerine ointment is commonly prescribed to relax anal sphincter muscle and alleviate symptoms. Anal fissure symptoms include pain and bleeding especially after a bowel movement.

A physician makes a diagnosis of anal fissure by visual inspection. Most anal fissures are located in the posterior midline. Anal fissures associated with crohn’s disease, leukemia and HIV are typically off the midline.

Anal fissure treatment starts with lifestyle changes including increased fiber in diet, use of laxatives and avoid straining. #### Anal Itching and Hemorrhoids (Almorranas in Spanish)
Persistent itching of varying intensity of perianal skin is known as pruritis ani. It is a medical condition that affects around 5% of the population. Patients with anal itching report a strong desire to scratch their skin. Repetitive scratching leads to worsening skin irritation, sometimes leading to ulceration.

Many factors contribute to pruritis ani. Excessive moisture especially during the hot and humid summer season of Houston greatly exacerbates anal itching. By the same token, chemical irritants present in certain laundry detergent, toilet paper or soap can cause anal itching. #### Are You Spending Too Much Time Sitting on the Toilet Seat?
Prolonged toilet sitting is a common habit among our hemorrhoid patients. Most patients with internal hemorrhoids reports sitting on toilet seat between 10 and 20 minutes. Prolonged toilet sitting increases pressure in anorectal veins and weakens pelvic floor muscles; two direct causes for hemorrhoid development and growth.

You can avoid prolonged toilet sitting by changing certain lifestyle and dietary habits that promote soft regular bowel movements:

Stay hydrated by drinking plenty of water especially during the hot Houston summer. Consume a fiber rich diet and supplement with Benefiber powder as needed to soften your stool. Soft stools prevent straining during a bowel movement and decrease toilet sitting time. Furthermore, avoid bringing your phone or a book to the bathroom. Plan to sit on the toilet for 3 minutes max. #### Does Gluten Insensitivity Cause Hemorrhoids?
Janice from Houston sent us this question: “I suffered with hemorrhoids for many years. Three months ago, I developed an anal fissure, and it was very painful. My friend who has gluten intolerance recommended I stop gluten. I did a gluten free diet for one month and the fissure healed. Does gluten irritate hemorrhoids and anal fissure? Thank you”.

Dear Janice,

The most cause for hemorrhoids and anal fissure is constipation, low fiber diet and sitting on toilet for long periods. Another common cause is increased abdominal pressure with pregnancy, obesity or heavy lifting. Gluten insensitivity and celiac disease affect the small bowel are not directly associated with hemorrhoids or anal fissure development. Gluten stimulates the immune system in the small bowel resulting in damage to small bowel lining. This causes nutrient malabsorption, diarrhea and flatulence. Changes in bowel habits secondary to gluten intolerance may exacerbate hemorrhoid related symptoms. Furthermore, damage to intestinal lining causes bacteria and toxins to leak inside the blood. The blood vessels that carry these toxins from the intestines to the liver for detoxification constitute the portal circulation. Increased blood pressure in the portal circulation is a direct cause of hemorrhoid development. There are however no published studies about the effect of gluten intolerance on portal blood pressure changes.

To summarize, hemorrhoids are dilated blood vessels of the lower anus. There are no published studies to evaluate the effect of gluten on hemorrhoid development or symptom exacerbation. However, if you experience consistent improvement in hemorrhoid related symptoms with a gluten free diet and worsening symptoms when you resume gluten intake, I recommend you adopt a gluten free diet. #### External Hemorrhoids
So many patients blame their symptoms on their external hemorrhoids when in fact it is their internal hemorrhoidal disease causing the bleeding, itching, etc.

We’ve always preached that patient histories are pretty much worthless when it comes to patients’ perianal complaints, including their description of “where” their symptoms are coming from.  That’s why we always encouraged folks to come in for an evaluation to see what the real story was with them.  So often, when patients blamed their symptoms on their external “stuff”, we could fix the SYMPTOMS by banding their internal hemorrhoids.

In our experience, so often when patients tell us that “my external hemorrhoids are bothering me”, their symptoms are coming from their internal hemorrhoids.  This is why, rather than telling our patients “We don’t treat external hemorrhoids”, our staff would recommend that the patient come in for an evaluation.  So often, these same patients have rashes, fissures and/or internal hemorrhoidal disease which actually is responsible for the patients’ symptoms, and which can be successfully treated.

Obviously, banding the internal hemorrhoids doesn’t do anything directly to the external hemorrhoids.  We explain to patients that internal hemorrhoid banding has no effect on the external tags that they may have. Also, even though some patients may think that their external hemorrhoids may have decreased in size after banding, they usually haven’t – these external hemorrhoids just tend to not distend as much as they did pre-banding.

That all being said, most of the time, “external symptoms” improve after internal hemorrhoid banding, despite leaving the tags, etc., alone.

#### Gut Microbiome and Colon Cancer
The link between western diet and colon cancer has long been established. The underlying mechanism of food induced colon cancer, however, has not been well understood. A new study titled “Gut Microbiome and Colon Cancer: A Plausible Explanation for Dietary Contributions to Cancer” attempt to shed some light on this issue. The study recently published in the journal of the American college of surgeons implicate the role of gut microbiome in colon cancer development. The authors start by discussing the role of Helicobacter pylori in gastric ulcer and gastric cancer. H pylori was the first microbe to be implicated in chronic human disorders leading to cancer. They then discuss the diversity of human gut microbiome and its impact on human physiology. Indeed, the microbial gene set is 150 times larger than the human gene complement. 160 species of microbes live in the colon and modulate our immune system. They affect a large number of diseases like rheumatoid arthritis, cardiovascular disease, obesity, irritable bowel syndrome, liver disease and even gut motility.  and colon cancer.

The authors hypothesize that gut microbial metabolites initiate colon epithelial damage at the DNA level leading eventually to cancer development. To test their hypothesis, the authors are evaluating differences in gut microbiome between patients with and without adenomatous polyps. While studies are still at their infancy, findings are emerging in support of shifts in gut microbiome composition towards higher production of pro-inflammatory substances like hydrogen-sulfide and bile acids. If this hypothesis is proven correct, a number of preventative measures, including dietary changes, can be employed to change the gut microbiome and prevent colon polyp and cancer development.

Increased estrogen and progesterone relax blood vessel wall and with increased blood volume, larger veins develop. #### Painful External Hemorrhoids
External hemorrhoids are dilated blood vessels beneath redundant skin folds located around the anal verge. External hemorrhoids are commonly associated with internal hemorrhoids and treatment of internal hemorrhoids, with rubber banding, leads to external hemorrhoid shrinkage. External hemorrhoids can cause itching, and the feeling of lumps around the anus. External hemorrhoids may also bleed but less so often than internal hemorrhoids. The most common complication of external hemorrhoids is acute thrombosis. Thrombosis occurs suddenly and spontaneously and results from a blood clot forming within the lumen of a dilated hemorrhoid blood vessel. The clotted blood vessel stretches the overlying skin resulting in a tender purple color lump at the edge of the anus.

Pain is severe. Successful treatment in the first 24 hours after pain onset is simple. It requires excision of the thrombosed blood vessel resulting in immediate pain relief. Excision can be performed in an office setting with local sedation. Sitz baths following surgery keep the area clean and promote healing. Patients with thrombosed external hemorrhoids who present late after onset of pain are best treated with hemorrhoid creams and Sitz bath. Excision of thrombosed hemorrhoid is not recommended 48 hours after onset of symptoms. It may in fact exacerbate rather than alleviate pain. Hemorrhoids are a major cause of blood in stool. Typically, blood is bright red and you notice it after a bowel movement. Other health conditions that lead to blood in stool include:

#### Conservative Treatment
Home remedies and creams
A number of home remedies like Sitz bath, apple cider vinegar and over the counter creams are available to help alleviate hemorrhoid discomfort. Sitz bath lower anal canal pressure and provide some relief. Creams containing steroids decrease inflammation and local anesthetic decrease pain. These interventions put a Band-Aid over the problem and don’t eliminate the underlying cause.

Fiber Treatment
Insufficient fiber in the diet can lead to constipation which may result in the development of hemorrhoids and/or anal fissures. Increased fiber intake has been shown to reduce constipation by softening the stool and increasing the regularity of bowel movements.

Soluble fiber (such as Benefiber®) is easy to incorporate into your diet as it is tasteless and can be mixed with food or drink. Begin with 1 tbsp once a day and then gradually increase to 1 tbsp 3 times a day over the period of a few days. Stir Benefiber® into 4-8 ounces of liquid (carbonated beverages are not recommended) or mix with soft food (hot or cold). Stir well until dissolved. Increase your fluid intake as necessary to ensure you are drinking 7-8 glasses of water every day. Supplemental fiber should be taken with meals for greatest benefit.

Many less-expensive generic versions of Benefiber® are available with similar active components.

#### Diagnosis
Diagnosis of hemorrhoidal disease starts in the office with a good history and physical exam. At the conclusion of office consultation, a number of diagnostic tests may be ordered to confirm the diagnosis.
Anoscopy
Anoscopy is a simple test that can be performed in office with no sedation. Anoscopy is the most accurate test to examine the anal canal and distal rectum. It helps visualize internal hemorrhoids safely and quickly.
Colonoscopy
A thin, flexible, lighted tube is inserted into the rectum and advanced through the colon. The test can detect inflammation, polyps, cancer, bleeding and internal hemorrhoids. Patient is sedated during procedure. Small tissue samples called biopsies may be taken if needed.

Inspection and Copies of Protected Health Information
You may inspect and/or copy health information that is within the designated record set, which is information that is used to make decisions about your care. Texas law requires that requests for copies be made in writing and we ask that requests for inspection of your health information also be made in writing. Includes psychotherapy notes.
Includes the identity of a person who provided information if it was obtained under a promise ofconfidentiality,

We can refuse to provide access to or copies of some information for other reasons, provided that we provide a review of our decision on your request. Another licensed health care provider who was not involved in a prior decision to deny access will make any such review.
Texas law requires that we are ready to provide copies or a narrative within 15 days of your request. We will inform you of when the records are ready or if we believe access should be limited. If we deny access, we will inform you in writing.
HIPPA permits us to charge a reasonable cost based fee. The Texas State Board of Medical Examiners (TSBME) has set limits on fees for copies of medical records that under some circumstances may be lower than the charges permitted by HIPAA. In any event, the lower of the fee permitted by HIPAA or the fee permitted by the TSBME will be charged.
Amendment of Medical Information

Accounting of Certain Disclosures
The HIPPA privacy regulations permit you to request, and us to provide, an accounting of disclosures that are other than for treatment, payment, health care operations, or made via an authorization signed by you or your representative. Please submit any request for an accounting to the person listed below.Your first accounting of disclosures (within a 12 month period) will be free. For additional requests within that period we are permitted to charge for the cost of providing the list.
